Millesimo extends along the right bank of the Bormida River in Liguria. The Bric Tana Regional Natural Park includes part of the municipal territory.
Millesimo was founded in 1206 when Enrico II Del Carretto surrounded the village and the local castle with walls.

Worth a visit is the Church of Santa Maria extra muros, built during the 12th century outside the walls of the historic center of the town. The Romanesque building with three naves was mentioned in a document of 998 named San Petri da Millesimo.
